Position Description
The Patient Assessment Instrument Coordinator is responsible for coordinating data collection on the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI). Responsible for ensuring all supporting documentation is present. Provides education to staff to ensure compliance with Centers for Medicare and Medicaid Services (CMS) guidelines. Coordinates all efforts for immediate issue resolution concerning Functional Independence Measure (FIM) and Quality scoring. Conducts ongoing review of IRF-PAI related data, and actively collaborates with coding, Nursing, Rehabilitation Therapy and Case Management to ensure accurate and timely IRF-PAI completion. Oversees the development of tools that assist in compliance and quality reporting for the Inpatient Rehab Unit (IRU).
Responsibilities
* Coordinates all areas of Functional Independence Measure (FIM) and Quality score capture and supportive documentation for the completion of the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI). * Reviews medical records to assure that clinical documentation supports FIM and Quality scores. Confers with the clinical team members regarding the scoring of FIM and Quality items to assist in resolution of any inconsistencies. * Reviews medical records and confers with medical staff members and coders to assure that co-morbidities are properly identified and documented in the medical record. * Provides information to case management regarding the CMG expected length of stay once all IRF-PAI admission information is inputted into system by day 3-4 of admit. * Reviews all Rehabilitation Impairment Categories and admission ICD-10 codes to assure congruence and proper Coding for the IRF-PAI. Makes any necessary changes in collaboration with the coder and physician. * Based on medical record review, assists in compiling data, tracking trends, and provides feedback to the clinical team regarding documentation, including providing input as to staff training needs. * Develops accurate monthly reports that identify trends in outcome measures. Provides outcome reports to IRU management and coordinates information for dashboards as needed. * Associate’s Degree or Bachelor’s Degree in a Clinical field * Minimum 2 years’ experience in Physical Rehabilitation field * Current State of Connecticut License in: Nursing, or Allied Health Profession * Prior experience with Functional Independence Measure (FIM) and Inpatient Rehab Facility – Prospective Payment System (IRF-PPS) preferred We take great care of careers.
