We are looking for a highly motivated individual who meets the following mandatory criteria: Qualified Professional (QP) Status:
Minimum of 2 years of experience
working with individuals with intellectual and developmental disabilities (IDD).
Education:
A
Bachelor's degree in Human Services
(or a related field) is required.
Travel & Flexibility:
Must be comfortable with regular travel between the Hickory and Shelby offices/client locations.
Schedule:
Standard work hours are
8:30 AM – 5:00 PM , Monday through Friday.
Skills:
Demonstrated strong organizational skills, an outgoing personality, and the ability to build positive relationships with clients, families, and team members.
Preferred:
Experience in supported employment or job coaching is a plus.
What You'll Do: Key Responsibilities As a Habilitation Manager, you will serve as a key internal case manager and client advocate, ensuring the highest quality of care and successful outcomes for individuals in our Habilitation program. Your responsibilities include: Client Advocacy & Planning:
Act as an internal case manager, collaborating with the Clinical Manager and actively participating in the Person-Centered Planning process.
Care Implementation:
Work closely with the client, family, Case Managers, Habilitation Technicians, and other teams to implement and evaluate client care needs and goals outlined in the Person- Centered/Individual
Support Plan (PC/ISP).
Oversight & Monitoring:
Conduct regular client visits to evaluate progress, ensure services are delivered according to the PC/ISP and service definitions, and track the progress of identified goals.
Team Leadership:
Provide consistent supervision of Habilitation Technicians in accordance with policy, procedure, and state regulations.
Communication:
Maintain continuous communication with Case Managers/Care Coordinators.
Team Support:
Participate in client-specific orientations as directed by the Clinical Manager.
