Description Conduct thorough evaluations in both inpatient and outpatient environments and create tailored treatment plans to meet individual patient needs Implement physical therapy techniques focused on enhancing mobility, strength, and functional capabilities Work collaboratively with nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes Provide education for patients and their families regarding therapeutic goals, discharge processes, and exercises for home practice Maintain accurate documentation of assessments, treatments, and patient progress as specified by regulatory standards Engage in interdisciplinary care team meetings to promote seamless patient care Education
Doctorate in Physical Therapy (DPT) or equivalent degree from a recognized accredited institution Certifications
Current state licensure as a
Physical Therapist CPR certification is preferred Specialized certifications (such as in neurology or geriatrics) are advantageous but not mandatory Skills
Previous experience in a related field is highly encouraged Expertise in managing patients with complex mobility and medical conditions Excellent communication and team collaboration abilities Proficient in using digital recordkeeping and clinical documentation systems Benefits
