Description Conduct evaluations for patients in both inpatient and outpatient environments and craft personalized treatment plans Implement physical therapy interventions directed at enhancing mobility, strength, balance, and overall function Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to maximize outcomes Inform and educate patients and families regarding therapeutic objectives, discharge planning, and home exercise regimens Maintain documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards Engage in care team rounds and interdisciplinary meetings to ensure comprehensive care delivery Education

Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution Certifications

State licensure as a

Physical Therapist CPR certification preferred Additional specialized certifications (e.g., neurological, geriatric) are beneficial but not mandatory Skills

Experience preferred but not necessary Ability to treat patients facing complex medical and mobility challenges Strong communication and teamwork abilities Familiarity with digital charting and clinical documentation systems Benefits
