RN Case Manager (Hospice) - Ocean, Monmouth County, NJ
Job Type: Full-Time (On-site, Homecare & Facility Visits)
Locations: Middlesex, Ocean, Monmouth County, NJ
Schedule: Monday-Friday, 8:00 AM-5:00 PM, with occasional night/weekend on-call needs (schedule flexibility required).
About the Role:
We are seeking experienced and compassionate Registered Nurse Case Managers (RNCM) to join a clinician-owned and operated hospice team dedicated to improving quality of life for patients and caregivers. This role is ideal for professionals passionate about providing care to individuals on their end-of-life journey, while working in a collaborative, supportive, and upbeat environment.
Key Responsibilities:
Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing patient evaluations. Initiate, develop, and update individualized plans of care according to patient needs. Collaborate with hospice physicians and the Interdisciplinary Team to carry out appropriate interventions. Supervise and coordinate the work of home health aides, social workers, and chaplains. Educate patients and caregivers regarding care plans, symptom management, and expectations. Maintain high standards of compliance in documentation and patient care. Support ongoing quality improvement efforts within the agency. Qualifications:
Active RN license in New Jersey. Minimum of 1 year RN experience (hospice experience is required) New Grad's are Accepted Willingness to travel within assigned counties. Valid driver's license, reliable transportation, and liability insurance. Benefits:
