Job Description American Traveler is seeking a Physical Therapist for Home Health assignments requiring home health experience and a current BLS certification.
Responsibilities Home health setting serving Sarasota and Charlotte County regions Covers patient populations including med/surg, cardiac, orthopedic, neuro, post-surgical, respiratory, and trauma cases Patient diagnoses include hips, knees, and shoulder surgeries, COPD, CHF, pneumonia, CVA, Parkinson's, weakness, failure to thrive, tracheostomy, amputation, and ADL limitations Skilled home care visits for Medicare, Medicare Advantage, and commercial insurance patients 6-8 patient visits per day, with a minimum of 6 points per day, which may increase at manager discretion Workday schedule is daytime, Monday through Friday, 8:00 am to 5:00 pm 1-2 weekend shifts per contract period are required Self-scheduled visits and hours depending on office needs Use of laptop, MiFi, portable printer, BayCare Forms and Scheduling, Ace Timesheet system for documentation Cerner charting experience preferred Responsibilities include assessment, treatment planning, documentation, and education for a range of complex patient needs in the home environment Will provide skilled visits and case management for diverse patient populations Solid color scrubs are permitted Pass/fail EKG exam is required within 3 attempts Must complete required online modules before starting; average completion time is 4 hours Smoking is not permitted in the facility or during work hours Return staff must wait at least one year before reapplying as a traveler, with exceptions for certain specialties No RTO (requested time off) is permitted in advance; all time off requests are managed directly with the unit manager on site
Requirements Active FL state license or compact license required if applicable Current BLS certification required Minimum 1 year of recent home health physical therapy experience required Valid driver's license and access to a reliable car required for home visits Must be COVID vaccinated or have a valid exemption form on file Must have flu shot or signed declination; mask required at all times if not vaccinated Experience with physical therapy assessments and treatments in cardiopulmonary, orthopedic, neurologic, and rehabilitation settings Knowledge of and experience with equipment such as gait belts, walkers, sliding boards, supplemental oxygen, wheelchairs, and assistive devices Familiarity with home exercise programs, patient and family education, medication management, safety and injury prevention, and abuse recognition Ability to float within the hospital or to affiliate hospitals as needed Compliant with 100-mile radius rule for eligibility
