Position Summary:

Key Responsibilities:
• Provide physical therapy services in skilled nursing facility setting • Deliver patient care according to established treatment plans • Maintain current state licensure and certifications • Complete required documentation and charting • Work day shift schedule from 08:00 AM to 4:30 PM • Collaborate with interdisciplinary healthcare team • Assess and treat patients according to physician orders • Maintain professional standards and facility policies • Participate in patient care meetings and discharge planning • Provide therapeutic interventions to improve patient mobility and function
Job Requirements:
Active, unencumbered license in NM
Minimum of 1-7 years of relevant experience, depending on specialty
Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
Current Resume listing detailed work history (MM/YYYY format)
Two current clinical references
State-required professional license
Physicial exam (completed within the past 12 months)
TB skin test or Quantiferon Gold (within the past 12 months)
Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
Current Tdap and Flu vaccinations
Must pass background check and drug screening
