Complete thorough patient assessments and documentation in
OASIS . Develop individualized treatment plans in collaboration with the care team. Deliver occupational therapy interventions to improve mobility, safety, and independence. Ensure timely communication and documentation in
HomeCare HomeBase (HCHB)
system. Travel to patient homes within the assigned territory (up to 100-mile radius). Maintain accurate mileage logs; mileage reimbursed at IRS standard. Uphold compliance with hospital, state, and federal regulations.
Required Skills & Experience: 1+ year of recent
skilled hospice/home health experience
(within the last 3 years; inpatient experience not accepted). Valid
Alaska OT license
and
NBCOT certification . Current
BCLS certification . Proficiency in

documentation and
