Position Summary:
The Occupational Therapist (OT) evaluates, plans, and provides rehabilitative services to patients with cognitive and/or ADL physical functioning disorders. The OT develops individualized plans of care in alignment with regulatory and clinical practice standards, utilizing evidence-based practice to improve patient independence, safety, and quality of life.
Responsibilities:
Screen and evaluate patients with cognitive/ADL functioning disorders and create individualized treatment plans.
Deliver rehabilitative, skilled, and medically necessary treatment interventions, including ADL retraining, therapeutic activities, exercises, modalities, and use of assistive devices.
Monitor and document patient progress, adjusting treatments as needed.
Evaluate outcomes and make referrals as appropriate.
Provide consultation and counseling to patients, families, caregivers, and service providers.
Collaborate effectively with supervisors and interdisciplinary team members.
Prepare comprehensive discharge summaries and maintain accurate patient records.
Document all required clinical and regulatory information; accurately track treatment minutes.
Participate in team meetings, case conferences, utilization review, and required facility/company trainings.
Supervise and support OT assistants, rehab aides, and/or students.
Adhere to NBCOT/AOTA and state practice acts, codes of conduct, and professional ethics.
Submission Requirements:
1+ year of OT experience –
Required
New grads –
OK
(must have completed Level II experience; Level I observation alone not sufficient)
LTC or Rehab experience –
Required
MO OT license –
