The San Francisco Campus for Jewish Living (SFCJL) is a center of excellence in providing a continuum of care on a nine-acre campus in San Francisco's Excelsior neighborhood. The campus includes the Frank Residences (luxurious assisted living and memory care), the Jewish Home and Rehabilitation Center (long-term skilled nursing, short-term rehabilitation, and an acute psychiatric unit for older adults over 55), and the Jewish Home and Senior Living Foundation. Grounded in the Jewish values of dignity, compassion, and community, SFCJL was founded in 1871 and continues to enrich the lives of older adults today. Admissions Intake Coordinator, Full-time 40 hours Schedule: Thursday Monday, 8 a.m. 4:30 p.m. (Days off: Tuesday & Wednesday) Location: San Francisco Campus for Jewish Living What Youll Do

Create admissions paperwork for all incoming admissions, including payer information (patient responsibility form) and general information about the Home. Create name tags and information sheets for incoming admissions, including Care Team contact information and Care Conference schedule. Run necessary screenings for potential admissions (e.g., Megans Law, Immunizations, PASSR, etc.). Complete admissions paperwork and upload appropriate documents to the EMR. Prepare and pass Admissions Memo to respective nursing units. Welcome newly admitted patients and assist them to their room; familiarize patients with call light, TV channels, phone, fan, and lighting. Provide general customer service to patients and families when needed. Take patient photographs and upload to the EMR. Provide tours of the units for potential patients and families. Assist team to answer call lights promptly and help patients get their needs met. Monitor cleanliness of F-building elevators and entryway floor; report cleaning requests to housekeeping and maintenance issues to maintenance. Why You Should Work for Us
