This role requires flexibility and supports the OptumCare Primary Care Neighborhood Program across three different facilities.
Primary Responsibilities:
Execute effective Case Management activities, leveraging strong clinical knowledge and a commitment to ongoing learning.
Utilize knowledge of Utilization Management and plan benefit design.
Demonstrate critical thinking nursing skills as an RN Case Manager, with the capability to organize and prioritize tasks effectively for yourself and your patients.

Required Qualifications:
Undergraduate degree or equivalent experience.
Current, unrestricted NV RN license or temporary NV RN license with plans to secure a permanent NV RN license.
3+ years of experience in direct patient care, especially in discharge planning or case management.
Strong verbal and written communication skills. Proficient in MS Office and other practice management systems, with a willingness to learn new applications.
Preferred Qualifications:
Bachelor's Degree in healthcare or related field, or actively pursuing one.
Certified Case Manager (CCM) credential.
Experience in case management.
Salary compensation will range from $71,200 to $127,200 annually based on full-time employment, influenced by local labor markets, education, experience, certifications, and other factors.
