Job Summary: We are seeking a licensed Physical Therapist to provide physical therapy services to participants in a clinical and home setting. The role involves traveling to various sites to deliver therapy, contributing to interdisciplinary team meetings, and developing personalized treatment plans.

Role and Responsibilities:

• Conduct assessments and provide physical therapy treatments.

• Participate in the development and implementation of care plans and quality improvement activities.

• Educate staff, participants, and families on therapy services.

• Supervise and train physical therapy assistants and restorative aides.

• Maintain accurate documentation and ensure confidentiality.

• Assess and recommend adaptive equipment as needed.

• Travel to various locations for participant home visits.

Requirements:

• Current Physical Therapist license in the state of California.

• Minimum of one year of experience working with a frail or elderly population.

• CPR and First-Aid certification.

• Valid California driver's license and a clean driving record for travel.
