VNA & Hospice of the Southwest Region of Rutland, VT is looking to hire per diem Licensed Nursing Assistant to service Bennington County. Responsibilities include but are not limited to: Under the supervision of a Registered Nurse and/or licensed Therapist, receives daily patient assignments and carries out assignments in accordance with individualized patient plan of care. Collects patient and family data and reports findings to RN and/or licensed Therapist in a timely manner. Assists in the communication of information to assigned patients. Under the supervision of an RN and/or licensed Therapist, performs interventions and procedures as directed to ensure the safe effective quality of care for patients. Reports to RN and/or licensed Therapist on a variety of aspects of patient and family information including. Advocates for patient care issues to ensure that overall quality and type of care is sensitive to each specific patient/familys needs. Performs a variety of patient care tasks including, but not limited to activities of daily living, collection of vital signs, ambulation/exercises, etc. Serves as Hospice Aide as directed. Serves as Personal Care Attendant as directed. Proactively reports changes in skin conditions to RN, Case Manager or program manager in a timely manner.
