The Program Coordinator, Maternal Child Health (MCH) Male Involvement is responsible for program coordination and administrative oversight of Westside Healthy Start male involvement activities. This position coordinates functions related to program implementation, data collection, and reporting. The role provides education, referrals, and support to expectant fathers and new parents in individual and group settings. This position requires travel to ACCESS health center locations — Austin, Humboldt Park, Madison, Sinai, Warren, Westside. Core Job Responsibilities

Coordinate the program development and design, implementation, and day-to-day operations at various Westside Healthy Start sites. Engage assigned or referred father/male participants in Westside Healthy Start male involvement case management curriculum, including assessing needs, developing service plans, and linking to external resources. Collect data and aid in development and maintenance of reporting tools to submit to internal and external venues. Implement and facilitate male involvement group education classes in accordance with grant requirements. Provide callback and other follow-up services to facilitate male involvement. Network and establish external partnerships to support effective external referral for males enrolled in the program (e.g., relationships with prospective employers pertaining to resources for employment). Develop activities of interest to engage male participants in co-parenting and child development activities. Create and distribute monthly reports detailing activities completed and outcomes according to grant and manager requirements. Conduct regular site visits with Westside Healthy Start Community Health Specialists to ensure co-parenting activities are successful. Other duties as assigned. Requirements / Preferences

Minimum Associate’s Degree in a health-related field (social work, psychology, counseling, rehabilitation, sociology, or other human service field). Bachelor’s degree or equivalent preferred. Minimum 3 years of work experience in a community-based or public health setting. Minimum 3 years of case management experience in a public or private social service program or health care setting. Intermediate proficiency with Microsoft Office products and database applications (e.g., Redcap, Electronic Medical Record). Minimum 2 years of project management experience with high-risk populations. CPR Card Required. Access is a network of Federally Qualified Health Centers treating patients on the frontlines of community-based health care. Depending on position applied for, candidates may be required to be vaccinated against communicable diseases and provide supporting documentation, or apply for religious and/or medical vaccination exemptions as part of the application process. The pay ranges provided represent the minimum to mid-range for positions.
