Duties

Conduct comprehensive foot and ankle examinations to diagnose and treat conditions such as fractures, infections, wounds, deformities, and chronic disease complications. Develop and implement treatment plans based on patient history, condition, and lifestyle. Provide wound care management, including diabetic foot ulcers, vascular issues, and infection control. Perform minor and corrective surgical procedures as indicated for foot and ankle pathologies. Prescribe medications, orthotics, prosthetics, and therapeutic interventions as clinically appropriate. Monitor and manage patients with systemic conditions (e.g., diabetes, vascular disease, arthritis) that impact podiatric health. Educate patients and families on preventive foot care, chronic disease management, and mobility preservation. Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated, multidisciplinary care. Identify and appropriately refer patients to specialists for advanced care when necessary. Maintain accurate and timely documentation in compliance with EMR/EHR systems, HIPAA, and organizational policies. Adhere to infection control and safety protocols when delivering care in varied home settings. Recognize and respond to urgent/emergent conditions, initiating escalation protocols when required. Travel daily between patients’ homes in Brooklyn; reliable transportation is required. Skills and Qualifications

Doctor of Podiatric Medicine (DPM) degree from an accredited program. Licensed (or license-eligible) Podiatrist in New York State. Valid DEA license (or eligibility) for prescribing medications. Board Certified or Board Eligible in Podiatric Medicine and/or Surgery (preferred). 1–3 years of clinical experience; prior home-based, community health, or hospital experience preferred. Strong diagnostic, surgical, and wound-care management skills. Proficiency in radiology for interpreting X-rays and other imaging studies. Proficiency in EMR/EHR systems and medical documentation. Excellent communication, interpersonal, and cultural-competency skills for working with a diverse patient population. Experience in dementia care as it relates to patient communication and understanding. Familiarity with systemic health issues (e.g., diabetes, vascular, gastroenterology) that may impact podiatric health. Experience working in or with a Level I Trauma Center (preferred). Ability to travel daily throughout Brooklyn with required medical equipment. Job Type
