in your community? Cheer Home Care is looking for an RN Care Case Manager who loves spending time with older adults and provide high quality care in home. The RN Care Case Manager collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, and nurses. They work closely with our office team and have excellent communication and problem-solving skills. This position will be responsible for serving as the primary care manager, coordinating care to the clients among RNs, therapists, and aides. Responsibilities
Serve as the primary care manager and coordinate care among RNs, therapists, and aides. Collaborate with interdisciplinary team members to develop and implement individualized care plans. Provide feedback to clients, client representatives, and nurses. Work closely with the office team; demonstrate excellent communication and problem-solving skills. Qualifications
Active California RN License, required Master in Nursing, preferred Industry Experience, preferred Upbeat Personality Strong Computer Skills Ability to multitask, drive, take action Strong Organizational & Communication Skills Pass Live Scan Fingerprint Clearance Valid driver\'s license and proof of insurance required Cell phone with Internet Access Proof of Negative TB results Willing to Work One Weekend a Month as Back-Up On Call Benefits
