Overview
Under general supervision, the Physical Therapist provides physical therapy evaluations and treatments to a diverse client population. Responsibilities
Perform physical therapy evaluations including assessment of functional status and physical condition Develop physical therapy treatment plans including goal setting Administer appropriate physical therapy interventions and procedures, including education and training of caregivers Communicate with other care team members with professionalism and respect Document per regulatory and Practice Act Guidelines Meet all administrative requirements including attendance of staff meetings and record-keeping Oversee care provided by physical therapy assistants in all settings and CHHAs in home environment Provide clinical case management and supervision over the home health care team when appropriate Participate in patient care meetings and performance improvement projects as assigned Explain treatment objectives and methods to patients and their families, including instruction in the use of prosthetic and orthotic devices and other special equipment and in procedures to be continued at home Instruct students-in-training and auxiliary personnel in elementary physical therapy procedures and techniques, and coordinate patient rehabilitation with physicians, nurses, teachers, occupational therapists, speech and language pathologists, and personnel in other departments Perform other related duties as assigned/required Qualifications
Registration as a Physical Therapist licensed by the Physical Therapy Board of California Basic Life Support for Professional Rescuer (BLS) Certificate issued by the American Heart Association or the ability to obtain within two weeks of hire and maintain throughout employment Eligibility for positions may require additional special conditions as per standard minimum qualifications Important Notes:
