Mass General Hospital is seeking highly motivated candidates to join the Cardiology Team. The Inpatient Cardiology Division includes the cardiac critical care unit, cardiothoracic critical care unit, two advanced general cardiology/step-down units, and one cardiothoracic step-down unit. This position is dedicated to cardiology clinical pharmacy services (general cardiology and critical care). The work schedule is five weeks of days, every sixth week of evenings, and every fourth weekend. Responsibilities

Provide safe, effective, and prompt medication therapy with direct patient-centered care and integrated pharmacy operational services in collaboration with physicians, nurses, and other hospital personnel. Align clinical services with target interdisciplinary programs and specialty services to deliver medication therapy management and ensure integrated pharmaceutical care programs. Assess patient needs, tailor drug therapy to age and disease-specific characteristics, educate patients, and adjust care based on patient response. Conduct practice-related research and participate in quality, safety, and regulatory initiatives to improve medication-use processes and pharmacy practice. Educate and train pharmacy staff and serve as a preceptor for pharmacy residents and Doctor of Pharmacy students (MGH Pharmacy Residency website: https://www.massgeneral.org/education/pharmacy-residency). Qualifications

Required Eligible for licensure to practice pharmacy in the state of Massachusetts. Board Certification by examination, or equivalent, in the relevant specialty is preferred; required within 18 months of hire. Preferred Completion of an ASHP-accredited PGY1 and PGY2 Cardiology Pharmacy Residency, Cardiology Fellowship, or experience as a cardiology clinical pharmacist is preferred. Essential Job Functions

Lead assigned patient service lines, clinical areas, and therapeutic programs to ensure drug utilization aligns with patient care needs and regulatory standards. Design and implement new pharmacy programs, quality improvement initiatives, and continuous quality assurance to optimize medication-use systems and patient outcomes. Lead stewardship activities and restriction/surveillance programs; track and evaluate programs for quality, operational, and financial efficiency; benchmark against best practices. Develop strategies to optimize medication utilization (clinical guidelines, disease-specific pathways, formulary management, and policies). Provide departmental and institutional leadership for committees, work groups, and external programs; ensure pharmacy is integrated within health-care delivery. Deliver direct patient care and clinical practice, including decentralized and service-based programs. Maintain proficiency in decentralized pharmacy services and patient-centered care; collaborate with multidisciplinary teams; establish collaborative pharmacist-patient relationships. Document patient information, collect medication histories, reconcile discrepancies, identify high-risk patients, and implement safety interventions. Provide evidence-based medication recommendations and manage care plans based on patient progress and goals; document in the medical record when requested. Maintain ACLS and BLS certifications; participate in medical emergencies; provide discharge medication review and counseling; educate patients and care givers. Remain proficient in hospital computer systems and medication ordering; interpret and process orders accurately; monitor and mitigate drug-related problems. Follow standard work parameters and maintain a safe, efficient work environment. Operations, Education, and Other Duties

Participate in pharmacy operations across central and satellite dispensing areas; oversee technician activities when assigned; promote formulary use and evaluate non-formulary requests. Precept and educate residents, students, and staff; develop training experiences and mentor learners to improve the medication-use system. Contribute to scholarship and professional activities through presentations, publications, and peer-review involvement. Additional Details
