The physician serves as medical director for skilled nursing facilities (SNF) and participates in administrative and quality initiatives for the facilities they oversee. Medical directorship functions include attending and participating in monthly quality assurance and process improvement meetings; guiding facility staff; reviewing and revising clinical guidelines; ensuring compliance with state and federal regulations; training facility staff; and supervising facility clinical staff. Serve as the attending physician for residents at each facility; collaborate with Nurse Practitioners and Physician Assistants to manage day-to-day resident care; typical facilities have a full-time PA or NP assigned depending on patient acuity and volume. Provide high-quality, patient-centered, comprehensive primary care services by rounding on residents in SNFs. Diagnose and treat acute illnesses (e.g., bacterial and viral infections) and chronic diseases (e.g., hypertension, heart disease, diabetes). Document in Theoria’s electronic health record (EHR) system, ChartEasy; work with other EHR systems such as PointClickCare, MatrixCare, and American HealthTech as applicable. Participate in Theoria’s Chronic Care Management, Transitional Care, RPM and other medical quality initiatives and programs. Supervise collaborating nurse practitioners and physician assistants; utilize electronic and state/county medical systems for administrative functions. Shift Structure
Shifts are flexible depending on physicians’ schedules. Typically, medical directors work a minimum of 2–3 days per week at each facility. Rounding times are flexible with no set start and end time; preferred start in the morning between 7–10 a.m. and finish when the caseload is seen. No nights or on-call; call is covered by Theoria's virtual care team. This is an in-person position. Compensation and Benefits
