Overview The Family Medicine Physician will provide primary care services in a community clinic setting, working with a multidisciplinary team to deliver comprehensive, patient-centered care in alignment with clinic policies and guidelines.

Responsibilities

Obtain medical, social and developmental histories.

Perform physical exams and immunizations on all patients.

Initiate laboratory examinations and studies as indicated.

Provide health education and counseling.

Manage acute and chronic illnesses to and above community standards at all times.

Consult with and refer to appropriate specialists for high-risk or complex patients.

Manage services in accordance with established clinic policies/procedures and contracted services under CHDP, Managed Medical Care and other medical programs and guidelines.

Exercise judgment in establishing priorities of medical care in triage situations.

Participate in the clinic’s Continuing Quality Improvement (CQI) program.

Provide case management with other health care team members.

Provide consultations to other physicians, nurse practitioners, physician assistants, and medical assistants.

Maintain a positive working relationship with staff.

Other duties as designated by the Medical Director or Executive Director.

Qualifications

Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited university

Active California Medical License (M.D. or D.O.)

Board Eligible/Board Certified in Family Practice

Active DEA License

Must have a Nat'l Provider Identifier (NPI), or obtain an NPI, prior to employment start date

Have approved credentials with Health Plans

Commitment to goals and philosophy of clinic

Current CPR and/or BLS Certification

Valid State Identification
