Job Summary

Hybrid schedule: 5 days a week, Saturday to Wednesday — 2 days working from home (weekend), and 3 days in the office. As a Clinical Coordinator at Elite Home Health Care, you will be responsible for overseeing nursing staff scheduling and assignments, maintaining internal visit tracking and discharge records, and completing daily patient activity reports. You will interact with nurses, families, clients, and caregivers to ensure timely nursing visits and provide completed aide supervisions to HR. Additionally, you will ensure timely and accurate documentation, processing, and billing of patient visits and records. Your meticulous attention to detail, problem-solving skills, and ability to work independently and as part of a team will be essential in effectively managing clinical operations and providing exceptional patient care.

Essential Duties and Responsibilities

Oversee nursing staff scheduling and assignments to ensure adequate coverage and timely patient visits. Maintain and update internal visit tracking and discharge records accurately and promptly. Complete daily patient activity reports to track patient progress and outcomes. Provide completed aide supervisions by nurses to HR as required. Interact with nurses, families, clients, and caregivers to ensure timely nursing visits and address any concerns or issues. Ensure timely and accurate documentation, processing, and billing of patient visits and records. Oversee and complete the creation and follow-up of 485 MD Orders when needed. Perform other duties as assigned by management to support clinical operations and patient care.

Qualifications and Skills

Meticulous attention to detail with the ability to work at a fast pace. Ability to problem solve challenging situations involving family members/clients, patients, and/or nurses. Ability to work independently and as part of a team, collaborating effectively with colleagues to achieve common goals. Excellent time management and organizational skills, with the ability to prioritize workload and adjust priorities as required. Customer service skills, including the ability to effectively communicate and build rapport with patients and their families. Ability to navigate and utilize electronic medical records and other software systems effectively.

Pay Range

The hiring range for this position is

$23.00-$26.00/hour . Various factors will determine final compensation such as a candidate’s years of relevant work experience, skills, certifications, and location.
