Responsibilities
Medical Oversight
Provide medical direction and oversight of the clinical aspects of the hospice care program, ensuring services comply with applicable state and federal regulations, including CMS regulations. Collaborate with the attending physician on admission and ongoing palliation and care of the patient\'s terminal illness as part of the Plan of Care (POC). If the attending physician is unavailable, address the medical needs of the patient.
Patient Care and Clinical Supervision
Review and help ensure implementation of patient care policies; tailor care plans to the medical needs of each patient. Provide clinical guidance to the interdisciplinary team and consult on complex medical cases to support optimal team functioning.
Compliance with Laws and Regulations
Ensure hospice program compliance with federal, state, and local laws, CMS, HHS, and Hospice Conditions of Participation (CoP). Collaborate with the Chief Compliance Officer to meet regulatory requirements of other bodies with jurisdiction over hospice services.
Education and Training
Provide education and training to hospice staff on medical care, policies, and regulatory compliance; participate in orientation and ongoing palliative care education.
Quality Assurance and Performance Improvement (QAPI)
Participate in QAPI activities, review patient care outcomes, and recommend improvements with the Chief Compliance Officer; contribute to policy development and revision for clinical and regulatory compliance.
Medical Certifications
Complete certification of terminal illness and recertifications as required by CMS and other agencies, in collaboration with the attending physician as appropriate.
Service Delivery and Visits
Provide services and visits to hospice patients according to the POC, with express authorization from Hospice; schedule visits through interdisciplinary meetings in alignment with the POC. Act as a medical resource for clinical staff and participate as a member of the interdisciplinary team to contribute to the patient\'s POC.
On-Call and Availability
Provide 24-hour on-call coverage for medical emergencies and address concerns outside regular hours; on-call scheduled with the Director of Clinical Services.
Compliance with Hospice Policies
Adhere to policies regarding personnel qualifications and patient care standards; ensure qualifications align with state medical board requirements and all services comply with regulations. Allow Hospice to perform criminal history checks for all employees or contractors with direct patient contact; allow OIG exclusion list checks monthly.
Record Keeping and Documentation
Maintain accurate, timely records of all services; documentation submitted weekly; ensure compliance with patient care requirements.
General Provisions
Participate in Hospice operations to improve patient care and clinical operations; provide leadership to maintain high medical care standards in accordance with law and best practices.
Pre-Admission and Care Coordination
Hospice will assess patients and families before admission for consultation; complete initial and comprehensive assessments if admitted; maintain continuous communication about medical condition and progress.
Patient Services and Case Management
Deliver services in a timely, safe, and effective manner in accordance with the POC and regulations; review and adjust care plans as patient conditions change.
Training and Education for Physician
Provide palliative and end-of-life care training during orientation and periodically thereafter to ensure high-quality care.
Qualified Personnel and Record Keeping
Ensure availability of qualified personnel; maintain compliant clinical records accessible for regulatory review.
Supervision and Evaluation
Supervise and evaluate the physician\'s activities; ensure attendance at IDT/IDG meetings and timely responsiveness to consultations.
Licensure and Qualifications
Physician: Maintain a current medical license in the state (or states) where the Hospice operates; promptly notify Hospice of any license revocation or restrictions. Hospice: Maintain a current state hospice license and Medicare certification; provide services in compliance with Medicare Conditions of Participation and state regulations; notify Physician of any license or certification changes. Kindful will run a state and national background check on all potential employees.
