The Harborview Medical Center Pharmacy Department has an outstanding opportunity for a Clinical Staff Pharmacist - Hematology/Oncology. Work Schedule
Full-Time / 40 hours per week Rotating/Variable Shifts This position is expected to last one year. Position Highlights
Act as an integral member of the HMC inpatient and outpatient Hematology - Oncology services, providing pharmacotherapeutic recommendations to primary providers, nursing and dietician staff to optimize patient care. Responsibilities include, but are not limited to direct patient care, drug distribution, research & education, and practice management as it pertains to the care of hematology-oncology patients. Primary Job Responsibilities
Develop and maintain policies and procedures for inpatient and outpatient hematology and oncology services. Assist in the development and carrying out of quality assurance programs for hematology and oncology pharmacy services, including ensuring organizational compliance with national practice standards (e.g. CORE measures, Vizient benchmarks, etc) Evaluate medication regimens for effectiveness, adverse drug events and patient side effects and intervene, as needed Provide patient counseling and education on self-management of disease and drug therapies Perform drug utilization evaluation and cost containment analyses, as needed Collaborate with providers to individualize drug therapies promoting safe, effective and economical patient care Assume responsibility for monitoring and modifying the drug regimen when referred, to include the ordering and interpretation of laboratory testing Identify barriers to adherence to medication regimen, incorporating patient characteristics i.e. language, culture/religious practices, physical, cognitive, emotional, pain/discomfort, socioeconomic status Function as patient advocate in order to justify therapy necessity or recommend alternative therapies when needed for insurance approval Work with the Specialty Pharmacy team to evaluate and further develop systems and processes. Participate as an active member of the hematology-oncology consult teams to assist in achievement of specific therapeutic outcomes by continuous monitoring of drug therapy Participate in all chemotherapy order review, clarification, validation, and order verification Qualifications
Strong candidates will have an advanced degree or residency, or 2 years of experience in clinically-oriented hospital pharmacy practice. PGY-2 oncology training preferred. REQUIRED POSITION QUALIFICATIONS State of Washington Pharmacist License and one year of experience in hospital pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the required experience. Legal Requirements
