As a Nurse Practitioner in our SNF Primary Care Program, you’ll collaborate with the interdisciplinary team to provide high-quality, coordinated care to long-term care residents. Your work will address patients\' clinical, functional, and psychosocial needs, with the goal of improving outcomes and reducing hospitalizations. This is an in-person position involving regular and clinically driven visits to patients, including follow-ups for changes in condition, treatment adjustments, and routine assessments. Key Responsibilities
Perform in-person clinical visits regularly and as needed based on patient condition, including acute concerns and follow-ups. Review and document patient medical histories and compile comprehensive diagnostic lists using evidence-based clinical judgment. Conduct clinical assessments of long-term care residents, considering complex comorbidities and frailty common in geriatric populations. Monitor and respond to acute changes in condition in a timely and appropriate manner. Collaborate with SNF interdisciplinary staff to develop and implement personalized care plans. Communicate changes in resident conditions and coordinate care interventions effectively. Monitor patient care dashboards and review daily condition reports to identify follow-up needs. Ensure accurate documentation of clinical encounters, including CPT and ICD-10 codes for billing and compliance. Adhere to facility protocols and documentation timelines. Requirements
Must hold current and valid license and registration as a Registered Professional Nurse and Nurse Practitioner in the state or practice. 3 years in clinical nursing practice in home care, skilled nursing facility or hospital. 3500 hours as a Nurse Practitioner, NPI #, and ability to bill both Medicaid and Medicare system with no restrictions. Experience working with Microsoft Office and working in electronic documentation EHR platforms. Job Type :
