The salaried GP will be responsible for delivering the full range of primary care services as part of the practice, ensuring the highest standards of care for the differing needs of our patient population. The salaried GP will work closely with the practice\'s multidisciplinary team of GPs, pharmacists, advanced practitioners, nurses and non-clinical staff to provide a high quality and accessible service to patients. It is expected that you will help support and input into clinical governance, good practice, quality outcomes framework, enhanced services, practice audit, referral review and other improvement plans as determined by the partners. The role may suit someone with specialist skills or someone looking to develop them, or someone interested in new ways of improving patient healthcare. The practice comprises Two Partners, Two Advanced Practitioners, two experienced nurses, three HCAs, an in-house pharmacy team and excellent administrative support providing care-coordination, care navigation and general reception services as well as data quality and management. The Lister Surgery & The City Medical Practice is looking for a qualified, enthusiastic, forward-thinking GP to join our teams. We will consider applications for between 6 and 8 sessions with a start date as soon as possible. You will be required to work across both practices, supporting our patients. We are rated Good with CQC and we value a good work-life balance with an open-door ethos. Main duties of the job To undertake duties governed by the PMS & GMS contract and Objectives of Direct and Local Enhanced Services including: Face to face and telephone consultations Patient home visits and nursing home visits Checking and signing repeat prescriptions Administration - dealing with queries, patient test results, referral letters NHS/private, tasks, emails and other paperwork and correspondence in a timely manner Cover all clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate Develop care and treatment plans in consultation with patients and in line with current practice disease management protocols as well as wider guidance such as NICE About us The surgery premises are situated in Westbourne Green Health Centre, 50 Heaton Road, Bradford, West Yorkshire, BD8 8RA. We operate in the inner-city area with an approximately 9,000 patient list. The team includes 2 Partners, long-term Locums, two Nurse Practitioners, two Nurses and 3 Healthcare Professionals. We are part of PCN4, providing ARRs roles integrated into practice roles such as MSK specialists, social prescribers and pharmacists. Our clinics include full diabetic patient clinics (including Level 2), women’s health and baby clinics, among others. The Practice Manager is trained in HR, IT and management reporting and has set up the websites and Teamnet systems. Job responsibilities Recording clear and contemporaneous IT-based consultation notes to agreed standards. Provide safe, evidence-based, cost-effective, individualized patient care within the surgery, patient’s home or other environment where care is delivered. Clinically examine and assess patient needs holistically, plan care accordingly, and refer to other services as appropriate. Recognize and perform necessary investigatory procedures, including laboratory and radiology results interpretation. Prioritize health problems and intervene in complex, urgent or emergency situations, including initiation of emergency care. Ensure appropriate follow-up of patients. Support patients to adopt health promotion strategies and self-care. Support anticipatory care plans. Maintain professional standards, documentation and confidentiality in line with policies. Independently prescribe within your scope of practice and in accordance with guidelines. Compile and issue computer-generated acute and repeat prescriptions. Review medications following policies, NICE/local guidelines and local care pathways. Stay aware of developments in clinical practice and comply with all relevant practice policies/guidelines (prescribing, confidentiality, data protection, health and safety, QOF). Be aware of duties and responsibilities regarding current legislation and safeguarding policies for children and adults.
