Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for a patient panel up to 450, recognizing that building a trusting relationship and supporting behavior change can influence outcomes.

Coaching for health: The PCP acts as a health coach, helping patients set short- and long-term health goals, partnering with them to achieve those goals, and frequently following up on progress.

Simplifying for action: The PCP prioritizes and simplifies to make behavior change actionable for patients and to lead care teams toward performance goals.

We are an outcomes-focused, value-based organization. For their panel of patients, PCPs are measured on metrics such as admissions per thousand, daily appointment slots (18–21 per day; each new patient counts as 2 slots, follow-ups 1), CGCAHPS, clinical gaps closures, and medical cost measures. PCPs will have goals for these metrics and work with center/market leadership and care teams to achieve them. Culture is important, and PCPs are expected to champion a positive culture of love, accountability, and passion along with center leadership. The PCP must demonstrate the ability to work independently and in collaboration with other health care professionals. The PCP will collaborate with managers and medical directors to ensure guideline compliance and participate in risk and quality management programs, clinical meetings, and other required meetings to promote patient health and company goals. The PCP will adhere to departmental goals/objectives, standards of performance, regulatory compliance, quality patient care standards, and policies and procedures. Essential Job Duties / Responsibilities

Independently provide care for patients with acute and chronic illnesses in older adults.

Take full accountability for patient care and outcomes and appropriately seek specialist consultations when needed while staying involved in the patient’s detailed care.

Engage with hospitalists whenever one of their patients is in the hospital, regardless of whether the hospitalist works for ChenMed.

Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs.

Lead the care team (care promoter, care facilitator, and care coordinator) for patients who can come to the office.

For patients unable to come to the office—hospital, SNF, LTC, or homebound—the PCP engages with the transitional care team and other resources available per market.

Active role in center management and coverage for other providers as needed; participate in recruiting patients and additional providers as needed.

Supervise, collaborate with, or function within a practice or agreement with an Advanced Practice Practitioner (APP) and remain accountable for the APP’s actions.

Performs other duties as assigned and modified at manager’s discretion.

Influence: competently and compassionately influence patients, teams, and themselves to achieve the best outcomes.

Self-care: prioritize physician wellness for sustainability and the health of patients, staff, and colleagues.

Proficient in MS Office (Word, Excel, PowerPoint, Outlook) and related software as used by the company.

Willingness and ability to travel locally as needed, and nationally for initial training and occasional recruiting or training purposes.

Fluency in English (verbal and written); other language fluencies may be required in some centers, disclosed at application time.

Ability to exercise independent judgment.

Education and Experience Criteria

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.

Must be able to obtain or hold an active State Medical License for the state(s) of employment.

Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar is preferred; board eligibility required.

Maintain board certification through MOC, CME, and/or retaking exams as required after board certification.

Current DEA number for schedule II–V controlled substances.

Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days of employment.
