Position Summary:

Key Responsibilities:
• Provide occupational therapy services in skilled nursing facility setting • Conduct patient assessments and develop individualized treatment plans • Implement therapeutic interventions to improve patient functional abilities • Document patient progress and treatment outcomes in medical records • Collaborate with interdisciplinary healthcare team members • Educate patients and families on therapeutic techniques and home programs • Maintain current state occupational therapy licensure • Adhere to facility policies and regulatory compliance standards • Participate in care plan meetings and discharge planning • Work day shift hours from 08:00 to 16:30
Job Requirements:
Active, unencumbered license in NH
Minimum of 1-7 years of relevant experience, depending on specialty
Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
Current Resume listing detailed work history (MM/YYYY format)
Two current clinical references
State-required professional license
Physicial exam (completed within the past 12 months)
TB skin test or Quantiferon Gold (within the past 12 months)
Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
Current Tdap and Flu vaccinations
Must pass background check and drug screening
