Occupational Therapist (OT) Home Health - Full Time / Part Time (Per Diem) in San Francisco County, CA. We are seeking a compassionate, organized, and team-oriented OT to join our Home Health team. This role may be conducted in the field with remote support as needed and reports to the Clinical Director. Location: San Francisco County, CA. Employment type: Full Time / Part Time (Per Diem). Position Overview: Evaluate patients, create and implement care plans, and track progress to maintain high standards of care. Collaborate with Clinical Director and Clinical Staff; work both remotely and in the field as needed. Provide, coordinate and oversee skilled intermittent care to patients in the home, including instruction and education of patients and/or caregivers to promote maximal functional status. Regularly evaluate and re-evaluate patient status. Administer treatments, monitor patient conditions, and report changes to the Physician, Case Manager, or Director of Nursing as needed. Initiate, develop, implement, and revise the patient\'s plan of care (POC). Address problems identified in the POC. Provide educational information to patients/family/caregivers regarding condition knowledge needs. Communicate with other disciplines involved in the case as warranted. Maintain accurate clinical records and submit documentation using a company-provided device. Comply with all company policies and procedures to ensure high standards of care. Requirements
Current OT License to practice in California (Required). Experience in Home Health or Hospice care (1 Year Required). Current CA Driver\'s License. Able to travel within a 15 - 25 mile radius on a regular basis. Benefits
