Required Qualifications

One year of recent experience as a Registered Nurse, including a minimum of six months in a Home Health setting as an RN Proficient in OASIS documentation Valid RN license in compliance with state regulations BLS certification (AHA/ARC) Preferred Qualifications

Home Health Nursing Certification (RN-BC) Other certifications and licenses may be required for this position Summary

A Home Health RN delivers direct nursing care to patients in their homes, ensuring comfort, stability, and effective management of medical conditions. RNs assess patient conditions, develop care plans, administer treatments, and educate patients and caregivers on disease management. Essential Work Functions

Assess patient conditions and develop individualized care plans based on medical needs and physician directives Administer medications, treatments, and wound care while monitoring patient responses Provide patient and caregiver education on medication management, disease progression, and self-care techniques Monitor for changes in patient status, report concerns, and adjust care plans as needed Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless care transitions Document patient care, interventions, and progress accurately and in compliance with regulatory standards Ensure compliance with infection control, safety, and home care best practices Support families and caregivers, offering guidance and emotional support Maintain communication with case managers and interdisciplinary teams for coordinated patient care Complete charting and documentation requirements while meeting visit and productivity goals Adhere to infection control, safety, and healthcare regulations Perform other duties as assigned within the scope of practice Required Essential Skills

Critical thinking, service excellence, and strong interpersonal communication Ability to read, write, and communicate in English Ability to follow written and verbal instructions and proficiency with PCs Physical ability to remain stationary, move about, handle equipment, push/pull, and bend Ability to work effectively with a variety of personnel and maintain professionalism Technical/motor skills to assist with procedures and use department equipment Ability to cope with frequent contact with the public, meet deadlines under pressure, and work with or without supervision as needed Visual acuity and effective communication skills Benefits
