Overview

Assistant Clinical Director (ACD) (LCSW) at The Child Center of NY oversees the engagement and support of Residential Treatment Facility (RTF) youth and families from entry through post-discharge, collaborating with the Clinical Supervisor for Therapy Services to center family involvement and secure entitlements, resources, and supports for positive post-discharge outcomes. Essential Duties And Responsibilities

Oversee the development, implementation and enhancement of structures and procedures of the Transitional and Permanency programs within the RTF, aiming to decrease length of stay, minimize re-referrals, and increase access to RTF treatment for those in the community. Provide ongoing clinical supervision and support to the transitional and family support teams, focusing on rehabilitative/clinical competencies, strengthening family relationships, educational success, and positive socialization. Assess family strengths and needs; develop and implement plans to address barriers to reunification, including progressive home time plans, skill-building/behavior management, caregiver support, and linkage to resources including social determinants of health. Lead joint meetings with The Child Center NY community-based programs to coordinate supports for families (Health Homes, CFTSS, Benefit Access). Ensure intensive community-based work with youth and discharge resources to support successful transitions, integrating RTF skills into discharge planning. Participate in clinical/treatment meetings (LOS, pre-admissions, CTPs, discharge conferences) and present weekly updates at Administration Meetings for the Transitional Coordinator Team. Maintain relationships with local resources and understand referral procedures and contacts. Support navigation of systems affecting youth and families (OCFS, DSS/ACS, DOE, Housing, Medicaid, etc.). Coordinate school supports with the in-house D75 school system and ensure regular contact. Collaborate with the Business Director to design and monitor transitional/wrap-around funds per OMH policies, ensuring proper use. Ensure compliance with all OMH regulations and guidelines. Provide Transitional Coordinator services to a caseload as required and on TC vacancies. Other related tasks as required. Supervisory Responsibilities

Backup to Director of Integrated Clinical Services. Supervise Transitional Coordinators, Permanency-Family Connections Specialist, and Intake Specialist in accordance with regulations and policies. Provide supervision and assess documentation compliance via chart reviews, community visits, and collaboration with Quality Specialist to ensure best-practice standards. Manage overall direction and evaluation of the Transitional, Aftercare and Family Support entities of the RTF Program and processes, focusing on community referrals. Serve as a key member of the RTF Management team. Education and/or Experience

Master's degree in a human services field with at least 2 years of direct service experience or linking children with SED to services. LCSW. Supervisory/managerial experience of at least 2 years in the social service sector. Strong written and verbal communication skills. Willingness to travel to families’ homes. Willingness to work at least one weekend day per week. Computer Skills

Knowledge of Microsoft Office Suite (Outlook, Word, Excel, PowerPoint). Other Skills, Abilities, Qualifications
