Physical Therapist Assistant: Home Health. Anticipated Territory: Santa Cruz County. Under the supervision of a Physical Therapist, you will assist in providing appropriate aspects of quality patient care and address the needs of individuals in their homes, so they can live safely and productively in their home environment. Under the supervision of a Physical Therapist, examine each individual and develop a plan using treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability. Work with patients to prevent the loss of mobility by developing fitness- and wellness-oriented programs for healthier and more active lifestyles. Qualifications & Requirements
License: PTA in California Home Health experience is highly preferred Current CPR Certification Current CA Driver\'s License Computer proficiency Strong organization and communication skills Ability to work independently and in a team Excel at building rapport with patients and providing high-quality care Experience with high-acuity patients is a plus Experience working with an interdisciplinary team is highly desired Case Management fundamentals is highly desired Self-directed, empathetic, critical thinker with outstanding interpersonal skills Leadership skills and strong organizational abilities Prior to scheduling an in-person interview, candidates may be asked to complete a computer literacy assessment What we offer

Base pay range:
$85,000.00/yr - $160,000.00/yr Estimated wage range: Physical Therapist Assistant (PTA): 85,000.00 - 160,000.00. Starting pay is commensurate with relevant experience. 21st Century offers generous bonus incentive plans, comprehensive health benefits, and 401k (up to 4% match). Requirements to Apply
