Overview
Older Adult Integrated Care Manager (QMHP). Location: Woodland Park Health Center, NE Portland, OR. Schedule: Monday through Friday, 8:30 a.m. to 5:00 p.m. Program: Integrated Team Based Care (ITBC). Cascadia’s Mission: Cascadia Health delivers whole health care – integrated mental health and addiction services, primary care, and housing – to promote hope and well-being. Position Description: The ITBC Older Adult Manager provides clinical oversight, support, and professional development for assigned staff. This role is part of a matrixed leadership structure and collaborates with ITBC leadership to create a robust system of support and accountability for all ITBC staff. It works with ITBC leadership to ensure program obligations comply with state licensing standards, county requirements, and internal policies. It also develops best practices and serves as the subject matter expert for assessments, treatment, harm reduction, case presentations, and diagnostic methodologies. The position promotes integrated care and trauma-informed, person-first approaches within a safety-focused, inclusive work environment. In collaboration with the leadership team, set annual goals and objectives aligned with integrated treatment and the agency’s mission; review and revise processes. Assist with the development of an operating budget to support program goals and objectives. Serve as Leader of the Day (LOD) weekly, handling client calls and crises. Provide resources and tools to supervisees to ensure quality work. Collaborate with leadership to develop clinical guidelines and operational workflows. Provide clinical consultation to the team as needed. Coordinate with the leadership team to offer one monthly clinical training for staff. Advocate and collaborate with leadership teams, executive management, and other departments. Attend internal and external meetings to problem-solve, advocate for programs, and maintain positive working relationships with employees and stakeholders. Oversee and leverage clinical data to monitor performance and assist in decision making. Serve as a liaison between departments to facilitate collaboration among interdisciplinary teams and communicate with stakeholders. Perform other duties as assigned. Regulatory & Compliance
Comply with laws, regulations, policies, and procedures under which the program and company operate. Comply with and manage relevant Contracts, Memorandums of Understanding (MOU), and Grants. Assist in crisis situations as appropriate; follow protocols with the treatment team. Respect client rights and responsibilities and demonstrate professional boundaries and ethics. Maintain accurate documentation ensuring compliance with agency policies and local, state, and federal regulations and billing requirements. Adhere to mandatory abuse reporting laws and HIPAA requirements. Understand safety regulations and evacuation procedures; participate in regular safety drills. Complete assigned employee training in compliance with program and company requirements. Provide guidance, support, and training to staff in alignment with the company mission and values. Participate in all scheduled staff meetings and supervision sessions. Supervisory & Leadership
Provide regular supervision, coaching, and annual training oversight for direct reports. Determine staffing requirements, recruit, hire, and train staff in alignment with Cascadia’s Values. Establish work plans and priorities, and assess progress of staff members. Promote and complete annual employee training requirements on a timely basis. Model and promote a culture that supports Cascadia’s Values and trauma-informed care practices. Provide guidance on SOPs, EHR, TIC, FIT, evidence-based practices, crisis management, harm reduction, OAR, HIPAA, and mandatory reporting. Participate in on-call rotation to provide after-hours support as needed. Qualifications
Education:
Master’s Degree in a behavioral science field from an accredited college or university. Must demonstrate the competencies required to become credentialed as a Qualified Mental Health Professional (QMHP) per Oregon Administrative Rules (OAR). Experience:
Required: Minimum five years in community mental health or related field; 3 years clinical experience providing community behavioral health services and a minimum of 2 years staff supervisory experience. Preferred: Clinical experience with older adults. Specialized Knowledge, Skills, & Abilities:
Understanding of the needs of individuals with complex medical conditions; recovery model concepts; motivational interviewing; illness management and recovery; evidence-based practices; strength-based case management; harm reduction; cultural responsiveness; trauma-informed care; and proficiency with relevant software/tools. Other:
Ability to commute to other locations. BLS/First Aid and AED certifications are required and must remain current. Working Conditions
Environment: Work in administrative and clinical settings, including client residences and community sites. Be prepared for interruptions to meal periods and potential safety concerns; follow universal precautions.
