The Care Coordinator role is a bilingual Spanish position in California within the remote Care Anywhere team. The Care Coordinator supports field providers, handles scheduling, outreach, and manages care coordination for high-risk members enrolled in the program. Schedule: Mondays - Fridays Responsibilities
Manage four provider schedules to ensure schedules are filled. Prepare charts for upcoming home visit appointments (check member eligibility, gather records needed by the provider prior to the home visit). Conduct outreach for scheduling, appointment confirmations, wellness checks for high-risk members, and to providers/pharmacies for member needs. Handle inbound/outbound calls (60 - 80 calls per day). Obtain medical records from provider offices, hospitals, and skilled nursing facilities (SNF) and upload to the EMR. Submit referral authorizations to IPA/medical groups for specialty, DME, and home health services. Coordinate lab orders and transportation for high-risk members. Document via EMR for inbound/outbound calls. Support SMS and member outreach campaigns. Assist the nurse practitioner (NP) team with visit preparation needs, including appointment reminders and assigning members to NP in the EHR, and providing needed documentation for visits. Direct inbound calls from members/family related to medication refills. Assist with maintaining and updating members’ records; mail or fax correspondence to primary care physicians or specialists as needed. Attend Care Anywhere meetings/presentations as appropriate; recognize work-related problems and contribute to solutions. Coordinate with outside vendors to provide appropriate care needs for members. Job Requirements:
Required: Minimum one year experience entering referrals and prior authorizations in a healthcare setting. Preferred: two years healthcare experience. Education:
Required: High School Diploma or GED. Preferred: Completion of medical assistant program from an accredited school. Training:
Preferred: Medical Terminology. Specialized Skills:
Able to communicate professionally with others; provide leadership, teach and collaborate. Knowledge of ICD-9 and CPT codes; knowledge of Managed Care Plans; able to type 40 WPM; proficient with Microsoft Outlook, Excel, Word; strong written and verbal communication; bilingual English/Spanish required; preferred: knowledge of Athena. Licensure:
