Responsibilities
Lead patient service lines, clinical areas, and therapeutic programs
Conduct in-clinic visits, follow-up monitoring, patient education, and medication reconciliation
Coordinate care for specialty medications to ensure access and affordability
Deliver direct patient care and participate in pharmacy operations
Provide education and training, serving as a preceptor for pharmacy students and residents
Qualifications
Graduate of an ACPE accredited School of Pharmacy
Completion of ASHP accredited PGY-1 Pharmacy Residency
Strong written and verbal communication skills
Significant clinical knowledge and expertise in patient-centered care
Eligible for licensure as a pharmacist in the state of Illinois or completion of the licensure process within 60 days of employment
Preferred Qualifications
Doctor of Pharmacy or another advanced degree
PGY-2 Pharmacy Residency preferred
2-4 years of clinical pharmacy experience in a hospital setting
Pediatric experience
