Overview

Care Manager/Designated Care Manager position at Sunrise on Connecticut Avenue, Sunrise Senior Living, San Rafael. The Care Manager is responsible for providing the highest quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The role involves hands-on care and support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe, home-like environment. The Care Manager demonstrates Sunrise’s Mission to champion quality of life for all seniors in accordance with applicable laws, regulations, and Sunrise standards, policies, and procedures. Responsibilities

Review, read, notate, and initial the Daily Log to document pertinent resident information. Participate in the development of ISPs and monthly updates; review designated assignments. Manage a designated group of residents during shifts, know resident locations, and conduct regular checks. Observe, report, and document changes in residents’ conditions (skin, behavior, alertness, weight, diet, activity participation). Attend daily Cross Over meetings with the lead care manager; notify supervisor of increased care needs or changes in condition. Respond to security systems and resident call bells promptly; greet guests, family members, residents, and team members; handle calls professionally. Assist with continence management and sanitation of resident suites and community restrooms per standard procedures. Communicate with families and serve as a resource as needed; help residents maintain independence and safety while honoring Resident Rights and Sunrise Principles of Service. Assist with activities of daily living (ADLs) and Invite, Encourage, and Assist (IEA) residents to participate in ISP-directed activities; engage in life skills and reminiscence activities. Promote empathy, warmth, and a calm environment; ensure safety regulations are followed; practice Standard Care Precautions. Host daily resident activities and participate in ongoing safety and infection control practices. Coordinate with the team to ensure compliance with regulatory requirements and safety procedures. Reminiscence Neighborhood Specific

IEA residents in life skills and daily routines; integrate life skills into daily activities; create multi-sensory experiences; participate in monthly letters to residents and families. Training and Teamwork

Contribute to team goals and uphold the Team Member Credo and Sunrise Principles of Service. Engage in engagement programs and scheduling meetings; participate in ongoing training with coordinators and attend required trainings to maintain Sunrise standards. Follow infection control policies and safety regulations; maintain required training compliance. Core Competencies

Ability to stay calm in difficult situations and manage multiple priorities. Strong written and verbal communication; good organizational and time-management skills. Good judgment, problem solving, and decision-making abilities. Qualifications

High School diploma/GED; may be required per state regulations. CPR and First Aid as required by regulations. At least 18 years of age; previous experience with seniors preferred. Desire to serve and care for seniors; ability to make decisions in the resident’s best interest. If applicable, drivers must sign the Driver job description and understand safety duties.
