Overview
Analyst, Case Management – Field – Must reside in 60010, 60067, 60089, 60102, or 60090. This telework position requires 50-75% travel to meet members face to face. Responsibilities
Facilitate appropriate healthcare outcomes for members by providing care coordination, support and education using care management tools and resources. Evaluate referred member’s needs/eligibility and recommend an approach to case resolution by evaluating benefit plans and available programs/services. Identify high risk factors and service needs affecting outcomes and refer to clinical case management or crisis intervention as appropriate. Coordinate and implement care plan activities and monitor progress. Enhance medical appropriateness and quality of care by consulting with case managers, supervisors, Medical Directors and other health programs; present cases at case conferences to achieve optimal outcomes. Identify and escalate quality of care issues; negotiate options and services; promote member engagement and healthy lifestyle changes. Coach and empower members to participate with their providers in healthcare decision-making. Monitoring, Evaluation And Documentation
Utilize case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures. Required Qualifications
Minimum 2 years of experience in behavioral health, social services or human services field. Minimum 2 years of case management experience. Must reside in 60010, 60067, 60089, 60102, or 60090, or surrounding areas. Willing and able to travel 50-75% of the time to meet members face to face. Preferred Qualifications
Discharge planning experience. Managed Care experience. Microsoft Office experience. Education
Bachelor's degree or non-licensed master level clinician required, with either degree in behavioral health or human services (psychology, social work, marriage and family therapy, counseling). Anticipated Weekly Hours

Full time Pay Range
The Typical Pay Range For This Role Is $21.10 - $44.99 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Benefits
