Primary Care Physician - Primary Care Plus - Houma, LA. Primary Care Plus clinics are part of a clinically integrated network designed to provide enhanced care to Medicare Advantage, Quality plan, and ACO members by delivering high quality, coordinated healthcare that produces superior outcomes in a cost effective manner. We are recruiting well qualified Internal Medicine and Family Practice physicians to join our growing practice in Louisiana. Our clinical team includes physicians, APRNs, RN Care Managers, clinic support staff, and a Quality Improvement team who work together to coordinate care for patients in both the outpatient and inpatient settings. A robust transitional care and high risk outreach program provides physicians with additional information to support their patients outside of the clinic. PCPlus uses a sophisticated electronic medical record system and robust analytical data analysis to identify patients who are high risk for poor outcomes to improve quality of life. Physicians are evaluated on patient outcomes rather than productivity or RVUs. Most physicians will care for panels in the 600-800 range and appointments are scheduled for 30-60 minutes per patient. The PCP is a Licensed/Board Certified/Board Eligible professional in internal or family medicine who provides direct patient care in ambulatory settings and participates in clinical rounds, documentation, and multidisciplinary collaboration to meet patient care needs and regulatory requirements. Responsibilities

Assisting in the development of the plan of care and providing counseling and education to patients and families. Ordering and interpreting laboratory and diagnostic studies. Ordering appropriate medications and treatments. Referring patients for consultation when indicated (e.g., dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social services, dietary, etc.). Documenting through in-depth progress notes and summaries. Participating in patient care rounds and conferences. Communicating patient management strategies to the patient care team. Collaborating with multidisciplinary team members to ensure effective patient management. Recognizing urgent situations and initiating life-saving procedures when necessary. Core Competencies

Doctor of Medicine (M.D.), Doctor of Osteopathy (D.O.), or M.B.B.S. Board certification or eligibility in Family or Internal Medicine Active, unrestricted medical license in Louisiana Valid DEA license Preferred Qualifications
