Overview
Reliance Hospice and Palliative Care is looking for Per Diem Hospice Nurse Practitioners to support our South OC areas such as Laguna Woods, Mission Viejo, Rancho Santa Margarita, San Juan Capistrano and San Clemente. Job responsibilities will include Hospice face-to-face visits, Palliative admissions and follow-up visits. $150 per visit for Initial Admission/Enrollment, $135 for follow ups. (Training will be paid $50 an hour) Responsibilities
Provide hospice services in compliance with the state Nurse Practice Act and under the supervision of a collaborating physician. Evaluate and assess the health history; diagnose medical conditions; manage the plan of care; prescribe medications or treatments; coordinate and collaborate with patients, families and other healthcare practitioners. Under the supervision of a physician, visit patients, complete physical assessments, perform face-to-face assessments and evaluate hospice eligibility as assigned in consultation with the hospice interdisciplinary team. Build from community resources to plan and direct services to meet the needs of individuals and families within their homes and communities. Hospice face-to-face visits, Palliative admissions and follow-up visits. Qualifications
Graduation from an accredited School of Nursing. Completion of an accredited Nurse Practitioner Program. Current nursing licensure in California and CPR certification. Master’s degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience. Accreditation from an approved certifying body for advanced practice nursing as required by State. Certification of Hospice and Palliative Nursing (CHPN), preferred. Excellent observation, verbal and written communication skill, problem solving skills, mathematical skills; nursing skills per competency checklist. Benefits
