Delivering high-quality outpatient primary care to a diverse patient population Managing preventive care, acute illnesses, and chronic conditions Collaborating with multidisciplinary teams to coordinate patient care Utilizing Epic and other digital tools for clinical documentation and patient communication Participating in quality improvement and community health initiatives Contributing to the growth and development of new and established practice sites Qualifications
Board certified or board eligible in Family Medicine, Internal Medicine, or Med-Peds 8+ years clinical experience preferred, with established patient panel a plus Strong leadership skills or experience managing patient panels Commitment to high-quality, patient-centered care Excellent communication and interpersonal skills Ability to work in both established and developing outpatient practice settings Demonstrated adaptability and teamwork Duties & Responsibilities
Deliver outpatient primary care to a diverse patient population Manage preventive care, acute illnesses, and chronic conditions Collaborate with multidisciplinary teams to coordinate patient care Use Epic and other digital tools for clinical documentation and patient communication Participate in quality improvement and community health initiatives Support growth and development of practice sites Compensation & Benefits
