Overview
Health Link Home Health and Hospice Agency is a renowned provider of comprehensive home health services serving the greater San Francisco Bay Area. We are currently looking for a compassionate, organized, and team-oriented Full-Time Licensed Speech Therapist (ST) for the San Francisco - Peninsula Area. You will work closely with our interdisciplinary team to ensure coordinated patient care and deliver outstanding services.
Responsibilities
Provide speech therapy services within the scope of practice and in accordance with the plan of care individualized for the patient, in coordination with the Interdisciplinary team.
Follow the plan of care; provide treatment; evaluate patient response to treatment and report any changes to the Physician, Case Manager, and/or Director of Nursing.
Maintain and submit accurate clinical records in accordance with organization policy.
Adhere to all company policies and procedures.
Other related duties as assigned in alignment with patient needs and program goals.
Qualifications
Current Speech Therapist License to practice in California (required).
Home Health experience (1 year) preferred.
Current California Driver's License.
Pay $60.00 - $75.00 per hour (depending on experience)
