Our Children’s Healthcare Center provides care for medically fragile and/or technology-dependent children, ages newborn through 21 years. Most children are admitted from a hospital NICU or PICU. This program focuses on patients who no longer require acute care, but who need significant medical attention related to wound therapy, PICC antibiotics, post-surgical care, NG, GJ tube feedings, and ventilator care. The Center is a “home away from home” for medically fragile children and their families. About The Nurse Practitioner Position

Provides support to the staff by following up on all clinic, hospital, and therapeutic visits and facilitating communication about all orders. Monitors all patient equipment to assure proper setting and compliance with all MD orders. Demonstrates clinical problem solving and critical thinking skills necessary to assure patient safety and staff support. Essential Functions

Follows up on all medical visits and ensures that all orders are followed and implemented; Evaluates medication and medical supplies and places orders as necessary; Assists with development and implementation of the plan of care for each patient; Completes daily bedside assessments; Attends patient care rounds and offers input to staff on each patient’s needs; Attends healthcare appointments with CHC patients as needed; Attends staff meetings and presents information and educational topics as requested by the Director of Nursing; Provides in-service training to staff and parents as it relates to patient care; Monitors patient equipment; Assists with implementation of safety strategies; Assists with resolving safety concerns with patients; Orders supplies for clients as necessary; Follows up on health issues with patients; Assists in implementing best practices on the unit; Assists with regulatory and accreditation compliance; Provides patient updates to DSCC, DCFS, and Public Health as necessary; Prescribes medication when necessary in collaboration with the Medical Director; Assumes on-call responsibilities every other week and weekend; Participates in continued performance quality improvement; and Other duties as assigned Education/Experience

MINIMUM REQUIREMENTS: Licensed in the state of Illinois; Certification and licensed to practice as a Nurse Practitioner in the state of Illinois; Credentialed to prescribe medications; Have or obtain CPR instructor status with the first 60 days of employment; Maintain licensure and CPR instructor status. Skills / Knowledge

Experience in pediatric patient care, pediatric intensive care, medically complex children, pediatric tracheostomy, ventilators, G-tubes, and other pediatric healthcare modalities; Experience with medically fragile children and with NICU/PICU patients in a hospital setting; Advance health assessment, pharmacology, and pathophysiology skills; Experience with pediatric ventilated patients; Knowledge and experience in childhood immunizations, growth and development, development screening, prescribing and monitoring medications, school physicals, and treatment of common illness. Physical Demands

Required to frequently sit, stand, bend, stoop and walk, sometimes for extended periods. Required to lift/move objects weighing in excess of 20 pounds. Must have visual, hearing, and learning capabilities sufficient to perform the essential functions defined above. Environment And Scheduling

Work is performed in designated administrative office settings. Must be flexible and available to work additional hours when necessary. May be scheduled to work on weekends and/or holidays Salary

Salary Rate:
