Overview
Spartanburg Regional Healthcare System is seeking a full-time Certified Nurse Practitioner or Physician Assistant to join our Oncology Survivorship Program. The APP will work collaboratively with a multidisciplinary team to provide comprehensive follow-up care, symptom management, and wellness support for patients transitioning from active cancer treatment to survivorship. Position Highlights
Monday-Friday 8a-5p with occasional event support after clinic No weekends, holidays or call Outpatient (Inpatient to provide support to Hem Onc as needed) Primary Location: Spartanburg & Pelham (Union and Gaffney as needed) Responsibilities
Survivorship Visits & Clinical Support Comprehensive Survivorship Assessment Risk-Stratified follow up and shared care Psychosocial distress screening and response Survivorship Care Plans (SCPs) Clinical management and coordination Program leadership and accreditation Education and communication Outcomes and quality Documentation and compliance Minimum Requirements
Minimum of 2 years of advanced practice provider experience in Oncology Strong clinical assessment and decision-making skills Excellent communication and interpersonal abilities Commitment to patient-centered care and health equity Contact Information
