Description

UCSD Department of Psychiatry is contracted to provide Transitional Case Management Program (TCMP) services for CDCR. TCMP services are provided for eligible incarcerated persons under CDCR and the State Department of Mental Health. Under close supervision, the incumbent will work full time (Mon–Fri, 8 hours per day; start/end times vary between 6am–5pm) at various CDCR correctional facilities and other regional institutions as directed. The Benefit Worker (BW) will manage full caseloads of incarcerated persons releasing from CDCR who are potentially eligible for benefit entitlements (Medi-Cal, Social Security, VA), diagnosed with HIV/AIDS or mental illness, or those needing community-based continuity of care after release. BW will determine potential eligibility, assist in benefit applications (Medi-Cal, Social Security, SSI, SSDI, VA benefits), and act as a liaison with institutional and parole staff for complex cases.

Responsibilities

Manage full caseloads of eligible incarcerated individuals transitioning from CDCR institutions. Determine potential benefit eligibility and assist in applying for Medi-Cal, Social Security, SSI/SSDI, and VA benefits. Coordinate with institutional and parole staff and participate in case management collaboration as needed.

Minimum Qualifications

Four years of related experience, education/training, or a Bachelor's degree in a related area. Eligible for clearance to visit and work inside CDCR facilities. Ability to perform intake evaluations. Assessment, problem-solving and critical-thinking skills. Ability to work collaboratively with a diverse client group. Ability to work in a fast-paced environment managing large caseloads independently. Strong computer literacy; experience with word processing, email, and internet resources for client care and professional development. Demonstrated accountability and organizational skills to prioritize workload and meet deadlines. Experience in professional case management and ability to work independently and in a team with attention to detail. Ability to write concise psychosocial notes, case summaries, and reports; strong written and oral communication skills. Experience working with community mental health resources, other professionals, correctional and parole officers on behalf of clients. Maintain strict confidentiality and exercise professional judgment in reporting information. Excellent interpersonal skills to build working relationships across varied stakeholders; ability to work independently and maintain professional boundaries. Knowledge of clinical counseling principles, methods, procedures, and standards.

Preferred Qualifications

Master’s degree in Social Work, Psychology, or related field. Knowledge of federal, state, and local health and welfare laws; experience assisting clients with SSA, VA, and Medicare/Medicaid. Experience in professional case management with parolees/ex-offenders diagnosed with HIV/AIDS and/or mental illness; includes psychosocial assessments, service planning, housing, referrals and follow-ups. Training and experience with motivational interviewing with incarcerated populations; understanding of co-occurring disorders. Direct service experience with state prison populations and knowledge of prison culture and risks.

Special Conditions

Employment subject to a criminal background check and CDCR clearance. Full-time work in a minimum through maximum security state prison; travel to various prison sites; potential overnight stays. TB screening annually; reliable transportation for work travel.
