Provide outpatient primary care to senior patients with a patient-centric approach. Lead care teams and collaborate with a supportive care staff to deliver high-quality primary care. Focus on comprehensive management of medical and social needs of patients with comorbid conditions. Qualifications
MD or DO degree Board certified/eligible in Internal Medicine or Family Medicine (required) Fellowship training in geriatrics is welcome but not required Active, non-probationary state medical license Strong collaboration skills and ability to lead care teams Commitment to patient-centric, high-quality primary care Passionate about caring for older adults and medically underserved populations Benefits
