The Clinical Pharmacy Specialist provides comprehensive pharmaceutical care to home infusion and specialty patients of all ages through medication order review, preparation, dispensing, documentation, monitoring, patient counseling, and medication reconciliation. The Clinical Pharmacy Specialist collaborates with patients, physicians, nurses, pharmacists, and other healthcare professionals to deliver safe, effective, and fiscally responsible patient-centered care. The incumbent will serve as a subject matter expert to support clinical program development, policy-writing, provision of education, committee participation, and scholarly activity.
Represents the organization and partners as a member of the clinical leadership team
Participates in departmental activities, staff meetings, system initiatives, quality improvement programs, and other projects as required or assigned. Routinely facilitates high-level discussion and best practice across the organization.
Maintains a well-rounded knowledge of all patient populations and facets of pharmacy practice while concentrating on areas of specialty.
Monitors the medication pipeline and anticipates changes in medication therapy and the potential impact it will have on the clinical and financial delivery of patient care.
Reviews and monitors medication orders for accuracy and appropriateness. Intervenes with the prescriber as necessary. Documents all interventions.
Prevents, detects, monitors, documents, and reports adverse drug reactions and medication errors. Makes medication-related patient safety a priority.
Actively listens and asks open-ended follow-up questions to ensure the true question is answered and any underlying concerns are resolved.
Travels among CarepathRx facilities and partners when necessary and as directed to support client education and implementation.
Provides training and education to pharmacy staff, pharmacy residents, and pharmacy students as assigned. Assists with precepting of experiential rotations for PharmD, RD, and other clinical clerkship students
Serves as a preceptor for the PGY1 residency program
Proactively identifies issues and collaborates with medical and nursing staff to establish or modify medication-related policies and procedures, protocols and order sets to support safe and effective medication practices and use
Attends and/or leads assigned multidisciplinary committee meetings.
Participates in and actively promotes departmental activities.
Completes all annual competencies as assigned.
Performs other duties as assigned.
Skills & Abilities
Excellent research and verbal/communication skills.
Excellent organizational skills.
Proficient in Microsoft office suite.
Requirements
Doctor of Pharmacy degree from an accredited school of pharmacy.
Registered pharmacist with Pennsylvania licensure; willingness to obtain non-resident pharmacist licensure and support clients in multiple states.
2+ years’ experience in a hospital or infusion setting preferred.
Completion of an ASHP-accredited residency and Certification by the Board of Pharmacy Specialties preferred.
