Psychosocial Assessment: Perform psychosocial assessments considering family, social, and medical history. Maintain current knowledge of medical and mental health diagnoses. Report suspicions of abuse and neglect to legally mandated authorities for investigations. Complete documentation with accuracy and clarity. Resource Support: Collaborate with the health care team to develop treatment plans and identify resources to support interventions. Maintain fluent knowledge of internal and external resources. Provide real-time accurate information to patient/family/caregiver about eligible resources. Evaluate effectiveness of interventions and adjust as needed. Psychosocial Interventions: Provide psychosocial services identified in the patient’s comprehensive plan of care. Offer supportive counseling to improve emotional coping and adherence to treatment. Educate patient/family/caregiver and health care team about treatment options. Provide self-management support for high-risk/complex patients to enhance home management of chronic conditions. Facilitate groups/programs for special populations as needed. Coordinate family-centered care team meetings and provide crisis assessment and interventions to restore equilibrium. Interdisciplinary Collaboration: Promote system-wide improvement and patient/family well-being. Serve as liaison between patient/family/caregiver and the medical team; advocate within the healthcare system and with community agencies. Facilitate communication to minimize fragmentation of services and empower problem-solving for care options and alternative plans. Collaboration: Partner with patient, family, and healthcare team to ensure safe transitions of care. Educate professionals and students on psychosocial aspects of care and the role of social work. Promote a positive work environment and participate in professional development and meetings as assigned. Job Qualifications

Master's degree in Clinical Counseling or Social Work 2+ years of related work experience LPCC or LISW license (Ohio) May require clinical license, certification or training Job Details
