Speech Therapist Clinical Fellowship (CFY) Year at 21st Century Home Health Services (21HHS). All opportunities at 21HHS require being in the field visiting patients in their homes. Remote/work-from-home positions are not available unless specifically noted. Responsibilities
Provide speech and language therapy services to patients in a home health setting for CFY-year candidates. Conduct comprehensive assessments of speech, language, voice, and swallowing functions. Utilize standardized tests and clinical observations to diagnose disorders. Develop and implement individualized treatment plans based on assessment results. Provide therapy for speech, language, voice disorders, and swallowing difficulties. Monitor patient progress and adjust treatment plans as needed. Educate patients and families about communication and swallowing disorders; provide home exercise strategies. Offer guidance on dietary modifications and safe swallowing techniques. Work closely with a multidisciplinary team and participate in patient care conferences. Maintain accurate records of assessments, treatment plans, and progress; prepare reports for healthcare providers and insurance companies as needed. Ensure compliance with relevant laws, regulations, and professional standards; participate in supervision with a licensed SLP and stay current with practice advancements. Qualifications
Master’s degree in Speech-Language Pathology from an accredited program. Temporary California state license or certification to practice as a Speech Therapist CFY. Strong communication, interpersonal, and organizational skills; ability to work independently and as part of a team. Valid California License. About 21HHS and Achievements
