Part Time / Weekend Physical Therapist Case Manager: Home Health Base pay range:

This role includes coverage in San Francisco County and related home health responsibilities. What you’ll do as a Home Health Physical Therapist Case Manager
Perform an evaluation in the home and develop a care plan, coordinating with Physical Therapy Assistants (PTAs) per agency and regulatory standards. Help patients with mobility, movement, and regaining independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure a high level of care. Evaluate and report patients’ progress to the physician (MD). What you’ll need / Qualifications
Current unrestricted CA Physical Therapist License Current BLS/CPR Certification Valid CA Driver's License Knowledge of local community resources Knowledge of development, human behavior, and cultural influences on health care Knowledge of state and federal homecare regulatory guidelines (preferred) Home health care experience (highly preferred) Experience with high-acuity patients (a plus) Experience working with an interdisciplinary team (highly desired) Case Management fundamentals (highly desired) Self-directed, empathetic, critical thinker with outstanding interpersonal and leadership skills Organized Why you should work with us

Results are confidential and used as part of the hiring decision. Wage range: Physical Therapist, Level 1: $120,000.00 - $135,000.00 plus $10K annual bonus, paid quarterly. Starting pay is commensurate with relevant experience. 21st Century offers bonus plans, health benefits, and 401k with up to 4% match. Typically hired as PTI; promotion to PTII with larger case load and autonomy includes minimum salary of $135,000 plus $20K bonus. Employment details
