Monday–Friday, 8a–4p/9a–5p, with 1 weekend per month and alternating holidays Part-Time Position:
3 varied days per week, 8:30a–4:30p, with 1 weekend per month and alternating holidays Sign-On Bonus:
Up to $10,000 Salary Range:
$119,971 – $153,900 (based on full-time employment) Position Summary
The RN Care Manager is responsible for conducting comprehensive patient assessments, coordinating multidisciplinary care, and managing safe discharge planning for all patient populations. This role ensures patients—particularly those at high risk—receive appropriate resources to prevent readmissions and achieve optimal outcomes. The RN Care Manager also performs utilization review activities in alignment with departmental policy and collaborates closely with healthcare providers, patients, and families to support care transitions. Key Responsibilities
Complete initial and ongoing assessments for patient care and discharge planning needs Coordinate multidisciplinary care within and across hospital settings Provide patient and caregiver education and arrange necessary aftercare services Collaborate with payers and ensure compliance with regulatory requirements Support department goals in length of stay, readmission prevention, and patient satisfaction Perform utilization review activities including payer reviews and admission clinical reviews Document and communicate patient information clearly and accurately Qualifications
BSN required (or MSN); NYS RN license required PRI Certification (within 3 months of hire) 3+ years of ED, critical care, or med/surg experience preferred Case management and utilization review experience strongly preferred Knowledge of healthcare financial, regulatory, and payer issues Strong computer and communication skills
