General Summary of Position
MedStar Health is looking for a
PT (Physical Therapy) Homecare Clinician PV (Per Visit)
to join our team! As a PT Homecare Clinician PV, you will provide evaluation and physical therapy treatment in accordance with agency standards, the laws and regulations governing the provision of physical therapy services in Maryland, District of Columbia, or Virginia, and other regulatory requirements.

Primary Duties:
Analyze and document patients and family responses to interventions. Evaluate measurable progress toward goals and revise the plan of care accordingly.
Identify and modify underlying factors that impede progress. Make recommendations for updates in the plan of care, utilizing knowledge of interventions and resources.
Establish and document a teaching plan for patients and families based on their level of knowledge, diagnosis, prescribed treatment, and available resources.
Communicate respectfully with patients, families, supervisor, peers, and other health team members (verbal, written, demonstration).
Establish visit schedules and inform patients, families, and team supervisors.
Implement effective physical therapy treatment plans with patients, their families/caregivers, physicians, and healthcare team, and modify as needed.
Conduct evaluations including interviews, observations, palpation, and specialized assessments such as goniometry, muscle strength testing, and sensory testing, using appropriate tools.
Independently implement and document care plans for patients with routine to complex problems, ensuring continuity of care.
Maintain clinical records, collect data, and prepare reports on activities, submitting admission records within 48 hours and daily records within 24 hours.
Manage caseload and productivity according to patient acuity levels.
Participate in case conferences, include patients and families in goal setting and care plan revisions.
Represent the department in community outreach efforts and participate in educational programs.
Perform assessments related to patient illness episodes to determine habilitation/rehabilitation needs, recommend adaptive equipment or home modifications, and identify discharge planning needs.
Perform procedures and modalities such as therapeutic exercises, mobility training, gait training, heat/cold application, and ultrasound.
Qualifications:
Bachelor's degree in physical therapy from an accredited school.
1-2 years of clinical experience.
License to practice Physical Therapy in Maryland, District of Columbia, or Virginia.
CPR certification.
Valid State Driver’s License with a dependable vehicle.
This position offers a hiring range of $72,758 - $123,260.
